How do you verify that a name change of a child was done legally?

Initially, the affidavit used to be copied unto the back of the birth certificate, but eversince they changed the type of document (in terms of document paper w/ watermark and seal), they also did not include the affidavit on the back of the new certificate. So how do I know my parents legally changed my name and it was court approved?How do you verify that a name change of a child was done legally?I would think that if the birth certificate has been changed the name is legally changed.



In any case, as long as you are not using a false name to commit fraud it is legal to use any name you want.
