At the time of divorce and in your divorce papers is should state that you will be returning to your maiden name. When that is put into writing and signed off by the judge that your divorce is final. You take your papers back to social security office and show that part of the divorce agreement states you are allowed to change your name back and the S.S. office will reissues and update your information once again. Then you do what you did when you changed it to your married name. You go to the DMV, bank and so and so and show your divorce papers.

If you did not do this at the time of the divorce, you will have to go back to court and ask permission to legally change your name.

Some states require a judges approval to change your name back. If yours doesn't, all you have to do is go to the social security office and provide them with the divorce paperwork when you file for your name change.
You have to petition the court for a name change and pay a fee. Its not a great deal but its a pain. I regret ever changing my name in the first place.
