I do know that in Canada you can legally change your name by applying to the government with your original birth certificate and a form outlining the change you'd like with a lawyer's signature, etc. and you'll get a name change certificate. For a child under the age of 16 he or she would need their parent's signed permission on the form. I hope this helps.
