Ask for office of the circuit court clerk. Get a name change form. Be sure it's for an adult if you're over 18. Fill out and get it notarized. If you're under 18 your parents' need to get their signatures notarized too (on the same form). Then take it back to the court house and about two weeks later it's a done deal.
